Pregnancy Follow-up

From the day your pregnancy is diagnosed, you should come to follow-ups at regular intervals for the health of you and your baby to be born. During these follow-ups, some tests are performed in order to learn whether your pregnancy is progressing in a healthy way, to understand some undesirable situations that may occur during pregnancy, and to diagnose diseases that may occur in the unborn baby.it will.

It is recommended that every woman who is planning a pregnancy or who is not protected should take 0.4 mg of folic per day before pregnancy, and those who have given birth to a baby with an anomaly called neural tube defect in a previous pregnancy or who are in the risk group should take 4 mg of folic per day.

1st visit: Doktor will measure and record your blood pressure and weight during your first visit. The tests you should have at this first visit are fasting blood sugar, blood group, hepatitis antigen and antibody (if you haven't had hepatitis vaccine before). If you have not had a smear test to screen for cervical cancer in the past year, your smear test will also be done during this examination. If your doctor deems it necessary, he or she may ask you to test for vaginal infection during the vaginal examination. If your blood group is Rh(-) and your spouse's blood group is Rh (+), it does not mean that you have a blood incompatibility; it simply indicates that you are a candidate for blood incompatibility. You should have the indirect Coombs test done to determine if the blood incompatibility has settled.

2nd visit: 11-14. You will have an ultrasound between weeks. In this ultrasonography, nuchal translucency measurement and the presence of nasal bone will be examined for Down syndrome screening. On the same day, PAPP-A and Free-beta HCG hormones will be checked in the blood for Down syndrome screening. Your blood pressure and weight will be measured and recorded.

3rd visit: 22-24. During your visit between weeks, your blood hemoglobin and hematocrit values will be checked. Iron pills are recommended for those with a hemoglobin value of less than 11 g to reduce the risk of possible and pregnancy-specific iron deficiency anemia. Another test to be performed at this visit is a detailed ultrasonography, in which all the internal and external organs of the baby are examined in detail, and the tissues of the heart are scanned. If deemed necessary by your doctor, uterine artery Doppler examination, which examines the blood flow in the uterine vessels, will also be performed during the ultrasonography at this visit. In addition, if your doctor deems it necessary, the length of the cervix will be measured vaginally. Your blood pressure and weight will be measured and recorded. 24-28. Şeker  loading test will be done between weeks. The purpose of this test is to screen for gestational diabetes, which occurs in 4% of pregnancies. Even if the blood sugar is normal at the first examination, pregnancy-related diabetes may occur. If these people are not recognized and treated appropriately, they may encounter problems such as the baby being too big and permanent diabetes after pregnancy. Your blood pressure and weight will be measured and recorded.

4th visit: At 32 weeks, an ultrasonography will be performed to evaluate the baby's development. Your blood pressure and weight will be measured and recorded.

5th visit : 38-39. This is the last visit to be made to discuss the estimated birth weight of the baby, the suitability of the bony roof and the mode of delivery at the gestational week, and to plan the birth, and ultrasonography and vaginal examination will be performed. If your doctor deems it necessary, you will meet with the anesthesiologist for painless delivery. Again, your blood pressure and weight will be measured and recorded.

The number of visits and tests summarized above are the things that should be done for pregnancies that go on their normal course. If there is a problem with the baby or mother in any of these tests, or if the doctor deems it necessary, the frequency of these visits and the tests to be performed can be increased.

Pregnancy Frequently Asked Questions

one.What awaits me in the first 3 months of pregnancy?Pregnancy is a physiological event in which there will be some changes in your body. It is not a disease. You need to take folic acid supplements in the first 3 months of pregnancy. This will protect your baby from the so-called neural tube defect. Apart from this, no additional vitamin or iron supplements are necessary unless your doctor recommends it. If you have vaginal bleeding during this early period of your pregnancy, consult your doctor.

2.When do the nausea go away?Every pregnancy has different symptoms. If nausea and vomiting prevent you from eating something and you are hungry, you should definitely go to the doctor. Serum ilr fluid and electrolyte supplementation should be done. Nausea disappears after 16 weeks of pregnancy.

3.Is sexual intercourse prohibited during pregnancy?Sexual intercourse is not prohibited unless you have a diagnosis such as a threat of premature birth or miscarriage, or unless your doctor allows it.

3.Is smoking prohibited during pregnancy?Definitely. Babies of mothers who smoke have a lower birth weight.

4.What is pregnancy poisoning?Pregnancy is when your blood pressure is above 140/90 mmHg. This may be due to pregnancy-induced hypertension or to what we call preeclampsia. In this case, the lives of both mother and baby are at stake. If you experience increased blood pressure, swelling in the feet or blurred vision during your pregnancy, consult a doctor immediately.

5.What is gestational diabetes?Does it do me any harm? Contrary to the false news in the media, there is no harm in having this test. 24-28. It is recommended to be done during pregnancy weeks. In what we call gestational diabetes,  large baby, fetal lung development lags behind, some  electrolyte problems are seen in your baby after birth. This can be predicted by doing a sugar test during pregnancy.

6.What is fetal nuchal thickness?  At 11-14 weeks of gestation, the nuchal translucency of the baby is measured ultrasonographically by trained specialists. Nape thickness of 3 mm or more is a precursor of Down syndrome or cardiopulmonary anomalies. In addition, Down syndrome is investigated by looking at the nasal bone during this period. Simultaneously, blood is taken from the mother and a double test is performed (maternal blood, fetal nuchal translucency and fetal nasal bone)

7.Will my baby be disabled?I'm worried. If one of the parents has a genetic disease or a consanguineous marriage, they should preferably receive genetic counseling before pregnancy. Couples without a known risk factor should have dual test at 10-14 weeks and fetal anomaly ultrasonography (detailed ultrasound) between 22-24 weeks. In detailed ultrasonography, measurements of your baby are taken one by one from the skull to the arms and legs, and around the abdomen. Since all organs complete their development in these weeks, the organs are examined one by one. Ultrasonography diagnoses 70% of fetal anomalies (not 100%).

8.What is amniocentesis?If there is a possibility that there is a baby with genetic problems in your double test, triple test or quad test results, fluid is taken from the baby's sac for diagnosis. This fluid is sent for genetic analysis. Amniocentesis has a risk of 1/200 fetal loss due to the procedure.  For this reason, if the risk ratio revealed in the tests is above 1/200, amniocentesis is recommended.

9.Is genetic diagnosis possible without amniocentesis?Fetal DNA can be checked in maternal blood. However, if it is risky, amniocentesis should be performed for a definitive diagnosis.

10.When are baby movements felt?It is felt in the 20th week in the first pregnancy, in the 16th week in the second pregnancy. In the last three months of pregnancy, at least 10 movements in two hours are expected.

11thWhat are the situations when I need to go to the doctor immediately during pregnancy?Vaginal bleeding, increased blood pressure, inability to feel fetal movements, leakage of water, pain in the groin and lower back.

12. Normal birth? Caesarean section?If you do not have any problems with your baby, you should definitely give birth naturally. When you enter the last month of pregnancy, your doctor will make a roof examination to see if it is suitable for birth. If it is suitable for delivery and there is no abnormality in baby weight, normal delivery is recommended.

13. What is preterm birth? Termination before 37 weeks of pregnancy. Premature babies need to be cared for in an incubator because their lung development is not complete. Again, intracranial hemorrhage and necrotizing enterocolitis in the intestines  may develop in premature babies. The most common cause of newborn deaths is premature birth. 

14.Can preterm birth be prevented?You should care about all groin and low back pain before 37 weeks and go to the doctor. In patients with preterm birth, it may be possible to prevent delivery by administering a lung-enhancing needle and certain drugs that prevent premature birth. It is important to detect early and be alert to uterine contractions .

15. What is developmental delay?It is the arrival of the baby in the mother's womb at least 2 weeks later than the known gestational week. Requires close monitoring. Amniotic fluid follow-up, fetal umbilical artery Doppler follow-up and ultrasonography follow-up should be performed.

16. What is amniotic fluid?It is the liquid your baby is in. This liquid protects the baby from the effects of the external environment. Lack of fluid is a serious problem and may sometimes require early termination of pregnancy. Fluid excess (colloquially called water kizi) is a problem as well as fluid deficiency.

17. What is NST?It is a device that records the heartbeat of the baby in the mother's womb. It measures both heart rate and uterine pressure. NST  follow-ups determine whether changes in the baby's heart activity are required or not. If the NST is reactive, that is, normal, this gives us a week of well-being.

18. What is color Doppler ultrasonography?It is a method in which the currents of the blood vessels going to the baby are measured. It is especially valuable in cases with developmental delay.
